As lender you may ratify this information for the collection of this or these accounts. Telephone Contact: As long as there is a balance due on this loan, lender may contact Borrower at any telephone number provided in the Credit Application or subsequently provided to the Lender, Customer Credit, to service or collect this loan.
An investigation consumer report including information as to your character, general reputation, personal characteristics, and mode of living may be made. It is your right under public law 104-208 to request in writing, within a reasonable time from today, a complete and accurate disclosure of the nature and scope of the investigation. We will furnish said information to you at your request.

NOTICE CREDIT HISTORY FOR MARRIED PERSONS
The Federal Equal Credit Opportunity Act forbids all creditors from discriminating against any applicant on the basis of sex or maritalstatus in any aspect of a credit transaction.
